Athletic Eligibility and Registration

Any athlete wishing to try out for a sport at and/or participate in any off-season activities MUST be:

  • Current DIAA Physical, AND
  • eligible according to all DIAA regulations and ASD policies

DIAA Physical (PPE)

Athletes are not permitted to participate in any athletic activity without a current DIAA Physical Form (PPE). DIAA has created a new PPE form. This will be the only form accepted beginning in May 2026. The new form is posted on the DIAA and school athletic websites. Please see the DIAA Medical Forms page for more information.

Forms must be completed after April 1 to be eligible for the following school year and are valid from April 1 through June 30 each year. Forms must be uploaded into Rank One. No paper copies will be accepted.

DIAA Regulations

Athletes must follow all DIAA regulations. Please note that enrollment in a school does not guarantee athletic eligibility. The following factors determine eligibility: attendance, residence, age, amateur status, transfer status and academics.

Athletes must be in good academic standing according to ASD policies and abide by all other District and school participation guidelines. These guidelines are shared by coaches.
